An exciting opportunity exists for a Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) with extensive advanced practise expertise in Palliative Care with a strong interest in Aged Care. In this role you will be providing advanced nursing assessment, diagnosis, care and intervention to patients and families within the scope of practice of a NP/TNP Palliative Aged Care Supportive Service (PACSS). You will also provide high level clinical nursing, leadership, strategic governance and management in establishing a model of care for those patients and their families in their transition to end of life care.

What you'll be doing
The Nurse Practitioner/Transitional Nurse Practitioner (NP/TNP) for the Palliative Aged Care Supportive Service (PACSS) will provide advanced palliative care expertise to older people residing in Residential Aged Care Facilities (RACFs) across the Mona Vale region. Working in partnership with the NSLHD Specialist Palliative Care service and the BRACE program, the NP/TNP will deliver expert clinical assessments and care planning. This role will collaborate closely with key stakeholders including HammondCare Palliative Care and General Practitioners, ensuring coordinated, person-centred palliative care for aged care residents. The NP/TNP supports the education and professional development of nursing staff, allied health, patients, carers, and families. A key role is to facilitate the transition of aged care residents to end-of-life care while coordinating with relevant NSLHD services.

 

Scope of Work:

  • Providing autonomous, evidence-based clinical care for patients in Residential Aged Care Facilities (RACFs)
  • Conducting comprehensive assessments to identify physical, emotional, and spiritual needs, and implementing advanced, holistic management plans
  • Deprescribing/Prescribing evidence-based therapeutic interventions tailored to patient needs
  • Leading advance care planning and optimizing symptom management across physical, psychological, social, and spiritual domains
  • Referring complex cases beyond scope to BRACE Geriatricians, Palliative Medicine specialists, and other healthcare teams
  • Acting as a liaison between community and inpatient services to ensure seamless care coordination
  • Providing support and coordinating of care for people approaching the end of life in RACFs as well as their families and care providers.
  • Facilitating early recognition of people approaching the end of life with complex palliative care needs
  • Providing mentoring and education to nursing staff and other health care providers

Candidates will need to meet the following criteria:

  1. Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
  2. Registered Authorised NP with APHRA that has completed the Tertiary requirements including a Master’s Degree (Nurse Practitioner) OR Registered Nurse willing to enrol in a Tertiary Master’s Program for Nurse Practitioner.
  3. Minimum of 5,000 hours of advanced nursing/midwifery practice during the last six years appropriate to the specialty area (or working towards within the next 3 years).
  4. Ability to independently assess, investigate and implement appropriate treatment for patients within advanced nursing practice guidelines.
  5. Demonstrated clinical competency in advanced practice management in Supportive and Palliative Care and specific interest in aged care.
  6. Demonstrated ability to work autonomously and have effective communication and negotiation skills.
  7. Demonstrated ability to work independently within the wider multidisciplinary team.
  8. Current Driver’s Licence
