What you'll be doing
The Senior Clinician Community Health will be responsible for providing support to patients with late stage chronic and complex health conditions including:
- care planning and advance care planning
- palliative care needs assessments and collaborative treatment care plans
- referrals to relevant internal and external health and community support providers
- education/support to other clinicians in supporting complex patient needs.
Selection Criteria
- Current Registration as an Occupational Therapist with the Australian Health Practitioners Regulation Agency (AHPRA) or eligibility for accreditation as an Accredited Exercise Physiologist with Exercise and Sports Science Australia (ESSA).
- Demonstrated extensive knowledge and experience working with consumers with chronic and complex illnesses, including demonstrated ability to conduct advance care planning discussions.
- Demonstrated experience of care planning and case management of patients in the community with chronic and complex illnesses.
- Demonstrated understanding of specialist palliative care, a palliative approach and community health services.
- Demonstrated high level of interpersonal and communication skills, including written, verbal and computer skills and an ability to work collaboratively in a multidisciplinary team.
- Proven clinical leadership capabilities with the ability to provide consultancy advice, education and support to individuals, their families, allied health and nursing staff, and other health professionals.
- Current unrestricted drivers' licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances and overnight stays.

Occupational Assessment, Screening and Vaccination against Specific Diseases
This is a Category A position, please read and understand NSW Health policy directive PD2024_015 All new employees must agree to comply with the requirements outlined in this policy.
