Prince of Wales Hospital - Nurse Practitioner/Transition Nurse Practitioner - Geriatric Outreach Service


We are seeking a highly motivated Nurse Practitioner (NP) or Transitional NP to join our dynamic team supporting aged care patients at Prince of Wales Hospital and with the Geriatric Outreach Service (GOS).

This is a rewarding opportunity to work collaboratively with multidisciplinary teams to deliver advanced, person-centred care to older adults—both within the hospital and through outreach to Residential Aged Care facilities. The role involves clinical assessment, treatment planning, and support for complex geriatric presentations, with a focus on reducing hospital admissions and supporting continuity of care.

The Aged Care Nurse Practitioner (NP)/Transitional Nurse Practitioner works within the Prince of Wales Hospital (POWH) Aged Care patients and with the Geriatric Outreach Service (GOS). The service provides advanced clinical assessment, consultation, referral pathways and development of management plans including (where appropriate) hospital avoidance strategies for older people in the community, including Residential Aged Care facilities (RACF), who are at high risk of emergency department presentation or readmission to hospital. This position is based at Prince of Wales hospital Geriatric department and includes both day and evening coverage during weekdays and weekends. 
 


Where you'll be working

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care.

Selection Criteria

 

  1. Registered Nurse with the Nursing & Midwifery Board of Australia including endorsement as a Nurse Practitioner or working towards same as per the NMBA Guidelines on endorsement as a Nurse Practitioner

  2. Post graduate qualifications at Masters level, in a NMBA approved program of study or relevant clinical specialty, or working towards.
  3. Demonstrated high level knowledge and advanced clinical nursing skills including comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a broad range of challenging, complex and changing situations
  4. Demonstrated understanding of the ‘Stroke care delivery system’ and ability to lead and implement changes and develop direction in clinical practice by the initiation, utilisation and review of evidence based research.
  5. Proven high level knowledge and expertise in complex client centred consultancy, practice development activities including mentoring, education, active participation in communities of practice, policy development and quality improvement.
  6. Demonstrated advanced clinical leadership role, negotiation, liaison, interpersonal and problem solving skills across the specialty of Stroke
  7. Demonstrated ability to work with a high degree of autonomy and accountability while also working collaboratively in a multidisciplinary and multi service environment.
  8. Current drivers licence (or a willingness to travel in accordance with the demands of the position)
