About the Role
As a Nurse Practitioner (NP) In our Palliative Care Service, you will bring high-level clinical expertise to support complex and evolving care needs. You’ll work across residential aged care, hospital in the home (HiTH), and community settings, including ambulatory clinics.
In this autonomous yet collaborative role, you will:
- Provide expert clinical assessments and implement advanced care plans
- Lead initiatives in evidence-based practice and clinical service planning
- Actively contribute to quality improvement and mentoring of staff
- Collaborate with the Community Palliative Care Team (CPCT) to enhance patient outcomes across care environments
What You’ll Bring
- Registered Nurse with the Nursing & Midwifery Board of Australia including endorsement as a Nurse Practitioner or working towards same as per the NMBA Guidelines on endorsement as a Nurse Practitioner.
- Advanced skills in diagnostics, assessment, referral, and treatment planning
- Strong background in palliative care models and interdisciplinary teamwork
- Leadership capabilities with a focus on education, mentorship, and service development
- NSW Class C Driver’s License
