Are you a passionate and experienced Nurse Practitioner looking to make a real difference for older people? Join our Residential Aged Care In-Reach (RAC-I) team and deliver advanced, patient-centred acute care that helps residents remain well in their familiar environment while reducing unnecessary hospital presentations across the Hunter region.

 

About the Role

As a Nurse Practitioner, you will work collaboratively with Geriatricians, Medical Officers, Nurses, Allied Health Professionals, residents, families and carers to deliver expert clinical assessment, diagnosis, treatment and care coordination within your advanced scope of practice.

You will play a key role in providing resident-centred acute care, research, education and supporting clinical excellence, and contributing to service development within an innovative and growing model of care.

 

About You

 

You are a motivated and dynamic clinician with:

  • Strong clinical expertise in aged care and/or acute care.
  • Excellent critical thinking and problem-solving skills.
  • The ability to work both autonomously and within a multidisciplinary team.
  • Exceptional communication and stakeholder engagement skills.
  • A commitment to clinical excellence and ongoing professional development.

 

Essential Requirements

  • Current registration as a Registered Nurse with AHPRA.
  • Current Nurse Practitioner endorsement, or substantial progress towards endorsement.
  • Minimum three years' full-time equivalent experience in an advanced practice nursing role.
  • Current unrestricted NSW/Australian Driver Licence.
