Join us as the Nurse Practitioner for Residential Aged Care In Reach
If you’re the kind of clinician who lights up when you’re helping older people stay well, stay comfortable and stay home, this gig is going to feel pretty special. This is a chance to work at an advanced level, stretch your skills, lead with confidence and be part of a service that genuinely makes aged care safer, smoother and more human.
Where you’ll be working
Newcastle Community Health Centre is the definition of community care done right. You’ll be surrounded by a friendly, multidisciplinary crew who cover everything from chronic disease and community nursing to mental health, physio, podiatry, counselling and Aboriginal health. It’s a super welcoming hub where people come to feel supported, heard and looked after.
The best part? You’re based right in the heart of Newcastle. Think easy public transport, on-site parking, great coffee around the corner and all the city convenience without the stress.
What the role looks like
You’ll be delivering advanced, clinically focused care to older residents living in aged care facilities across Newcastle, Lake Macquarie, Port Stephens and Taree. Instead of sending residents to hospital, you step in with expert assessment, timely interventions and collaborative decision making to help them stay well in their own environment. You’ll work closely with Geriatricians, Medical Officers, fellow nurses and allied health teams to bring the best of acute aged care straight to the resident’s doorstep.
Expect a blend of virtual care, in-person visits, rapid assessments and truly meaningful conversations with residents, families and carers. You’ll be the steady, knowledgeable presence they trust during the moments that matter.
Requirements
- Current authority to practice as a Registered Nurse with AHPRA and current endorsement as a Nurse Practitioner or, if transitional, evidence of substantial progress towards endorsement
- The equivalent of three (3) years' full-time experience in an advanced practice nursing role within the area of intended practice, within the past six (6) years from the date when the completed application seeking endorsement as a Nurse Practitioner (Masters of Nursing-NP) is received by the Board.
- Valid unrestricted drivers’ licence for use in NSW/Australia
Why you’ll love it
This role gives you autonomy and a strong support system. You’re backed by the Director of Nursing Integrated Care Services, the Geriatric Medicine team, the HNELHD Nurse Practitioner Network and a local governance group who’ll help you shape your scope and grow your expertise. Orientation is provided, ongoing education is encouraged and you’ll be part of a service that’s still evolving, which means you get to help build its future.
