About the role
Join our dedicated team as a Nurse Practitioner – Chronic Care and play a key role in delivering advanced, evidence-based care to patients across the Forbes community. In this autonomous and rewarding role, you will provide comprehensive assessments, diagnose and manage complex and chronic health conditions, prescribe appropriate treatments, and work collaboratively with multidisciplinary teams to achieve the best possible health outcomes.
This is an exciting opportunity to provide clinical leadership while supporting quality improvement, education, mentoring and evidence-based practice in a rural healthcare setting. This position is a Lachlan Health Service position and is in Forbes. Lachlan Health Service incorporates both Parkes and Forbes and staff work across both sites when required.
