As the Clinical Pharmacist you will assist in the provision of a comprehensive clinical pharmacy service – you will assist across MLHD to provide education and informed suggestions in line with relevant pharmaceutical regulations.

About the role:
We are looking for an experienced Pharmacist to join us in this exciting role to provide pharmaceutical education and information to other health care professionals along with providing medicines information to patients and carers.
*Please note that we have both on site and fully remote roles on offer
As the Pharmacist, you will be responsible for:
Providing medication-related education to medical and nursing personnel, and to patients/carers.
Ensuring that medical and nursing personnel are fully aware of NSW Health initiatives & legislation and educate and encourage practice of these initiatives/guidelines.
Undertaking a medication history of patients on admission, review patient medication charts at each facility, perform medication reconciliation, and prepare a medication management plan (MMP) for acute patients as deemed appropriate.
Providing discharge counselling to patients/carers to ensure a full understanding of the medications prescribed

About You
Our ideal candidate will demonstrate:
Hold current AHPRA with the Pharmacy Board of Australia
Ability to participate in teaching programs with other pharmacists, university students, nursing staff or other health care providers, and ability to work effectively as a member of a multidisciplinary team.
Experience in contributing to quality assurance activities.
Ability to participate in a 7 day per week roster and willingness to drive to District facilities for work purposes if required
