Where you'll be working
This position is based at 670 Hunter Street, Newcastle, and forms part of a multidisciplinary healthcare team delivering services through the Hospital in the Home (HITH) program. The HITH service enables eligible patients to receive hospital-level care in the comfort and safety of their own homes, reducing the need for inpatient hospital stays while maintaining high standards of clinical care.

The team consists of a range of healthcare professionals who work collaboratively to provide coordinated, patient-centred care. As part of this service, the Occupational Therapy team plays a key role in assessing patients' functional abilities, supporting independence, promoting safety within the home environment, and assisting with rehabilitation and recovery goals.

The department is committed to delivering high-quality, evidence-based occupational therapy services to patients and their families or carers. This includes comprehensive assessments, intervention planning, equipment prescription, home environment recommendations, education, and ongoing support tailored to individual needs. Clinicians work closely with medical, nursing, allied health, and community care providers to ensure seamless care and positive patient outcomes.

The opportunity
As our newest Occupational Therapist within the Hospital in the Home (HITH) service will provide high-quality, patient-centred care to patients receiving hospital-level treatment in their homes. The role involves conducting initial assessments through home visits and virtual care, developing treatment plans, and supporting patients to maintain independence and safety within their home environment. Key responsibilities include equipment prescription, recommending home modifications, functional assessments, patient and carer education, and ongoing monitoring of patient progress. 
The clinician works closely with the multidisciplinary team to coordinate care and achieve positive patient outcomes.  The position requires effective clinical decision-making, caseload management, and accurate documentation. The role reports to the Occupational Therapy Team Leader or Allied Health Manager and may provide support or supervision to students as required. Support will be provided via Allied Health Service Manager and Occupational Therapy Head of Discipline.

Requirements

  1. Tertiary qualification in Occupational Therapy under the NSW Health Service Health Professionals (STATE) Award NSW and corresponding registration with Australian Health Practitioner Regulation Agency (AHPRA) and minimum 3 years full time clinical experience
  2. Eligibility to drive in NSW and willingness to travel for work purposes which may involve driving long distances.
