inTouch is a community-based service within a multidisciplinary team that provides virtual care and remote monitoring, working with medical specialists and other subject matter experts for escalation or consultation. The inTouch Service is onsite at Blacktown Hospital Campus.

What you'll be doing
As the inTouch Clinical Nurse Specialist 2, you will provide advanced, holistic nursing care to patients at risk of hospitalisation due to complex health needs. You will combine high-level clinical expertise with strong care coordination and leadership capabilities to deliver safe, proactive, patient-centred care in a virtual environment.
Your key responsibilities will include:
· Conduct comprehensive health assessments using advanced clinical skills
· Coordinate and manage complex care plans, discharge planning and escalation pathways
· Deliver self-management support and education to patients and carers
· Provide clinical leadership, mentoring and competency development for nursing staff
· Drive quality improvement initiatives and support implementation of new care models

Essential Criteria:
· Registration: Current Registered Nurse Division 1 National Registration APRHA
· Licence: Driver’s Licence
Selection Criteria:
1. Current registration with AHPRA as a Registered Nurse Division 1 with post registration qualification relevant to specialty and at least 3 years' experience working in the clinical area of the post graduate qualification or such other qualifications or relevant equivalent experience. Relevant qualifications include – Aged Care, Advance Care; Virtual care, telehealth and / remote monitoring, Urgent Care, Sub-acute Care, Digital Health.
2. Demonstrated knowledge in initiating and implementing changes to improve clinical service delivery and outcomes, models of care, evidence-based practice and practice development.
3. Demonstrated ability to communicate effectively with patients, carers and a multidisciplinary team, diverse range of health care providers including those in government, non-government and community to identify and improve opportunities to enhance continuity of care.
4. Demonstrated excellence in written and verbal communication skills and computer literacy in virtual care platforms, ability to use remote monitoring devices and evidence of personal time management capabilities and effective negotiation skills.
5. Current valid driver’s licence within Australia and willingness to use for work purposes.
6. Demonstrated ability to be flexible and adapt to changing role requirements as the service develops operationally.
