Registered Nurse: $79,368.82 - $111,427.34 per annum

In addition to base salary, all roles attract 12% Superannuation + Salary Packaging

South Western Sydney Local Health District is seeking a motivated intake coordinator to join the community-based Bowral Transitional Aged Care Program (TACP) as a Level 3 clinician in a full-time permanent position. TACP is a specialised program focused on the reablement of older adults living in the community. It delivers short-term, goal-oriented care aimed at improving health outcomes and maximising quality of life for individuals transitioning back into the community following a hospital admission. In this role, you will work collaboratively with the multi-disciplinary team and relevant stakeholders to provide holistic, person-centred care to TACP clients and their families.
As part of this position, you will be responsible for conducting assessments in the inpatient setting to determine the care needs of the client, organise appropriate services and equipment upon discharge and providing information to inpatient teams to raise awareness of TACP. You will also contribute to quality improvement and evidence-based practice initiatives within the team. With support from other clinicians, there will be opportunities to further develop your clinical reasoning, participate in service development activities, and engage in ongoing professional development, supervision, and clinical education.

- Has experience in aged care and/or rehabilitation
- Is able to work both independently and collaboratively as part of a multidisciplinary team
- Communicates clearly and effectively with clients, families, and team members
- Demonstrates problem-solving and clinical reasoning skills
- Complete comprehensive assessment of suitability to TCP for older patients being discharged from hospitals have been referred to the Transition Care Program
- Develop and monitor appropriate care plans to maximise improvement for clients via the delivery of low intensity, therapeutic intervention and in home practical support, with clearly identified and measurable functional goals.
- Provide case management support to clients of TCP to ensure there is coordinated delivery of therapeutic and in home support services at the client's home
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
- Recognised degree in a health related field (Nursing or Allied Health Professional) with current authority to practice as a Registered Nurse or Allied Health (OT, PT, SP, SW, Dietitian) with Australian Health Practitioners Regulation Agency (AHPRA) and where registration not required is eligible for membership with relevant professional association.
- Demonstrated recent post-graduate clinical experience in aged care settings.
- Demonstrated skills in the coordinated assessment and case management of older persons with complex care needs, including skills in cognitive assessment, health assessment, community support and residential placement.
- Demonstrated effective communication, interpersonal skills and time management skills to appropriately engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
