Delivering acute care in the patient's own home - Hospital in the Home (HITH) treats acute patient in the home and in the clinic, enabling the patient to recover in their own home and with their family and carers.

Where you'll be working
We are a district wide service and support our patients who live across our district. Our team are located primarily at Royal North Shore, Mona Vale and Hornsby. This role will be located at RNSH.
What you'll be doing
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
- What is your understanding of the HITH model of care and what your role as a Social Worker would be.
- Consider the impact and value of visiting acute care patients in their own home and how they interact with their social environment and respond to treatment themselves.
- What would be the key considerations for a social worker who works in a team delivering acute care - the impact on the person and their family/carers.
