Where You’ll Be Working
The Hospital in the Home (HITH) team is a fun and skilled multidisciplinary service made up of Medical, Nursing and Allied Health professionals who deliver care directly to patients in their own homes. Working from the Newcastle Community Health Centre, the HITH teams across the district provide care in the community, supporting timely recovery at home, reducing length of stay, and helping patients avoid unnecessary hospital admission. It is an incredibly important service for our district! As a pharmacist, you’ll be part of a service that values strong communication, shared decision‑making, and making sure we are delivery the best care for our patients and their families
About the Role
As the HITH Pharmacist, you will play a key role in ensuring safe, effective, and timely medication management for patients receiving care in the community. This district wide role supports four Hospital in the Home (HITH) services; Newcastle, Maitland, Manning and Tamworth. You would provide virtual linkage across the district to ensure patients receive seamless care wherever they are.
Your day-to-day work will include:
• Providing clinical pharmacy input for patients receiving acute care at home
• Conducting medication reviews, reconciliation, patient education, and safety assessments
• Supporting patients following discharge, to make sure this is a smooth transition, and they have the support they need
• Collaborating closely with the wider HITH team
• Ensuring continuity of medication supply and monitoring, including high risk medication, in the home environment
This role offers a unique opportunity to combine clinical pharmacy expertise with innovative models of care outside the traditional ward setting.
About You
Registered as a practicing pharmacist under the Australian Pharmacy Board with the Australian Health Practitioner Regulation Agency (AHPRA) and able to practice independently.
Hospital clinical pharmacy experience and knowledge of the processes governing the conduct of clinical trials with a sound understanding of the regulatory environment in this field.
Ability to manage a team and work collaboratively with other stakeholders as well as independently.

You will be well supported in this role:
• Receive a full orientation to the team and position
• The Service Manager will be help with day to day operational support and guidance
• Working closely under the guidance of the Medical Director, with clinical governance structures to support you
• As part of the wider HITH Multidisciplinary Team you’ll be welcomed into the team and work collaboratively with nursing, medical and allied health colleagues
• You’ll have access to experienced clinical pharmacists through the HNE Pharmacist Network. You can chat to colleagues for clinical advice, peer support, and professional development
• You will have the chance to link into education, pharmacy groups and professional development opportunities
