What you'll be doing
As the Hospital in the Home (HITH) Clinic Lead CNS2, you will provide clinical leadership within a busy and dynamic HITH clinic, ensuring the safe, efficient and patient-centred delivery of care. Drawing on your advanced clinical expertise, adaptability and sound understanding of patient flow, you will coordinate daily clinic operations, prioritise patient care activities, and support timely access to treatment across the HITH continuum.
Working collaboratively with medical officers, nurses and the multidisciplinary team, you will lead patient assessment, care planning and coordination, while managing competing clinical demands in a fast-paced environment. You will play a key role in optimising patient journeys, supporting effective resource utilisation, identifying opportunities for service improvement, and ensuring high standards of clinical care and patient experience are maintained.
This role is ideal for an experienced nurse leader who thrives in a complex and evolving healthcare environment, demonstrates strong clinical judgement and decision-making skills, and is passionate about driving excellence in Hospital in the Home care through innovation, teamwork and patient-focused outcomes.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Current registration with AHPRA as a Registered Nurse Division 1, with post registration qualification relevant to specialty and at least 3 years’ experience working in the clinical area of the post graduate qualification or such other qualifications or relevant equivalent experience.
- Demonstrated ability to communicate effectively with a diverse range of healthcare providers, including government, non-government, residential aged care, and community organisations, supported by excellent written and verbal communication skills, proficiency in Microsoft Office applications, strong time management capabilities, and effective negotiation skills.
- Current NSW driver’s licence.
