About the Opportunity
SWSLHD Transitional Aged Care Program (TACP) is seeking a temporary part time Team Leader (Level 4 Allied Health Clinician - Occupational Therapist, Physiotherapist, Dietitian, Speech Pathologist, Social Worker, Registered Nurse) to support their Liverpool community team in providing high quality care to older adults who are accessing the program. 

The Team Leader is responsible for the day to day operations of the Liverpool TACP team. This includes maintaining occupancy of the program, onboarding suitable clients, providing case management to aged care clients and leading a team of allied health clinicians & registered nurse. 

This position is for 16 hours per week (Monday & Friday) for 12 months and Office is located in Braeside Hospital.

What You'll be Doing
Coordination of a Transitional Care multidisciplinary team and provision of case coordination for older people who have functional impairment and high care needs discharged from hospital and eligible for Transitional Care Program.  
The position is responsible for service provision and discharge planning for TCP care recipients, as well as operation supervision and management of a multidisciplinary team of Allied health clinicians.

  1. Recognised degree in a health related field (Nursing or Allied Health Professional) with current authority to practice as a Registered Nurse or Allied Health (OT, PT, SP, SW, Dietitian) with Australian Health Practitioners Regulation Agency (AHPRA) and where registration not required is eligible for membership with relevant professional association.
  2. Demonstrated recent post-graduate clinical experience in aged care settings
  3. Demonstrated skills in the coordinated assessment and case management of older persons with complex care needs, including skills in cognitive assessment, health assessment, community support and residential placement
  4. Demonstrated experience in team leadership, with the capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise
  5. Demonstrated effective communication, interpersonal skills and time management skills to appropriately engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs
  6. Demonstrated experience working as a member of a multidisciplinary team
  7. Demonstrated ability to prioritise and manage a caseload
  8. Demonstrated computer literacy skills with reference to data collection, email and word processing
  9. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
