Sydney Local Health District (SLHD) is seeking a dynamic GP and Primary Health Network Clinical Partnerships Coordinator to join our team at Croydon Health Centre!

 

Are you an experienced Mental Health Clinician with demonstrated experience in developing strategic responses to the development of care pathways across diverse service delivery locations?

 

The priorities of the role are:

  1. Managing and promoting partnerships with the Primary Health Network and General Practitioners (GPs).

  2. Managing the GP/Mental Health Shared Care Project.

  3. Provision of clinical leadership to the Mental Health Shared Care Coordinators.

 

This innovative position is open for multiple grades including Clinical Nurse Consultant Grade 2 and Allied Health Level 4.
