About the Opportunity

Armidale Rural Referral Hospital is a 99-bed facility providing acute healthcare services to Armidale and surrounding communities, offering a diverse and rewarding clinical environment.  In this role, you'll work across a variety of units within the hospital, collaborating with patients, families and multidisciplinary teams to support effective discharge planning and continuity of care. 

You'll be joining a supportive team committed to delivering person-centred care and positive patient outcomes. The successful candidate will be supported by the Deputy Director of Nursing and Midwifery, with opportunities to further develop their clinical, communication and care coordination skills. 
 



What You Will Be Doing

You will work under the direction and supervision of a Registered Nurse to support the delivery of safe, evidence-based and person-centred care. 

A key focus of the role is coordinating discharge planning throughout a patient's admission, ensuring their changing needs are identified and addressed to support a smooth transition from hospital to home or other care settings.

In this role, you will:

  • Coordinate discharge planning from admission through to discharge.
  • Liaise with community services and external providers to facilitate ongoing care and support.
  • Conduct post-discharge follow-up as required.
  • Collaborate closely with multidisciplinary teams to assess, plan, implement and evaluate patient care.
  • Facilitate referrals to appropriate community and support services.
  • Support patients and families to understand and participate in discharge planning processes.
  • Contribute to positive patient outcomes through effective care coordination and communication.
     

About you

You are a compassionate and organised Enrolled Nurse who enjoys supporting patients throughout their healthcare journey. You thrive in a fast-paced environment, balancing multiple priorities while maintaining a strong focus on patient-centred care and positive outcomes. 

With excellent communication and interpersonal skills, you build strong relationships with patients, families, community providers and multidisciplinary teams. You are proactive, adaptable and confident coordinating care across a range of clinical settings. Most importantly, you are passionate about helping patients achieve a safe and successful transition from hospital to home.

Essential requirements:

  • Current registration as a Enrolled Nurse with AHPRA  

  • Demonstrated ability to work effectively within a multidisciplinary team.

  • Strong communication, organisation and time management skills.

  • Commitment to delivering safe, evidence-based and person-centred care.

  • Ability to work under the supervision and direction of a Registered Nurse.
