Enrolled Nurse – Transition to Residential Aged Care (TRAC Unit)
Our 20-bed TRAC unit is a unique environment where acute care nursing meets residential aged care. We support patients who have recovered from their acute medical episode but are no longer able to safely return home. At this pivotal stage in their lives, we work closely with patients, families, and carers to facilitate a smooth and dignified transition into long-term aged care.
Are you a compassionate and dedicated Enrolled Nurse with a passion for aged care? Join our supportive and dynamic team within the Transition to Residential Aged Care (TRAC) Unit at Belmont Hospital.
About The Role:
As an Enrolled Nurse in the TRAC unit, you will work under the direction and supervision of Registered Nurses to provide holistic, patient-centred care, including:
- Geriatric clinical nursing care
- Palliative and end-of-life care
- Emotional and social support for patients and families
- Assisting with care planning and transition processes
You will play an important role in supporting patients and their loved ones through what can be a challenging and emotional life transition, ensuring care is delivered with empathy, respect, and professionalism.
- Enrolled Nurse with current AHPRA registration
- Strong interest or experience in aged care, gerontology, or palliative care
- Excellent communication and interpersonal skills
- Compassionate, patient-focused approach
- Ability to work collaboratively within a multidisciplinary team
