What you'll be doing

Provide efficient and effective early intervention family focused recovery assessments, counselling and care coordination services for women and families accessing inpatient and outpatient Maternity and Child & Family Services. 
Provide hospital inpatient support where required and capacity permits.

  • Qualifications in Social Work with current registration with Australian Association of Social Workers registration.
  • Demonstrated application of professional and ethical boundaries to the delivery of patient care in the context of a multi-disciplinary team.
  • Demonstrated application of evidence based practice and critical thinking to advocate for and implement individualised person centred care patient care.
  • Demonstrated effective interpersonal skills, excellent written and verbal communication skills to work in a personable and professional manner and personally resolve conflicts when they arise.
  • Knowledge of the complexity of socio-economic issues impacting on health outcomes in rural communities with a demonstrated commitment to work and partner with Aboriginal and Torres Strait, Islander patients and families to enhance health outcomes for communities.
  • Evidence of participation in own professional development and commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise with colleagues.
  • Evidence of sound knowledge in Primary Health Care Models.
