About HammondCare
HammondCare is an independent Christian charity that exists to improve the quality of life for people in need. We specialise in palliative care, aged and dementia care, mental health services for older people, rehabilitation and other related health and aged care services. HammondCare seeks to embed evidence-based best practices in its services and we provide these services through home care, sub-acute hospitals and residential aged care.
Bringing these health, hospital and aged care services together, HammondCare has been able to develop innovative, flexible care models designed to serve people with complex health or aged care needs, regardless of their circumstances.
HammondCare works to provide its staff with support, career development opportunities, education, responsibility and recognition, work/life balance, and satisfying roles.
About the role
HammondCare is dedicated to improving the quality of life for people in need. Our Dreams program is designed to fulfill the wishes and dreams of our clients, providing them with memorable experiences and moments of joy.
The Dreams Program supports people with a life limiting illness to focus beyond their diagnosis and treatment, to a positive event or outcome that has personal meaning to them and funded by donations through HammondCare Foundation. The program acts as a vehicle of 'meaning' for clients with terminal illness and their families, with therapeutic value to support end of life, grieving and remembrance.
The Program Lead for the HammondCare Dreams program will be responsible for overseeing the planning, execution, and evaluation of the program. This role will be responsible for identifying appropriate team members required to implement each project and coordinate logistics around this. This role will also be required to work closely with Health Executive and Foundation Teams to drive awareness of the program throughout HammondCare and lay foundations for expansion into new areas of the organisation.
