Salary per week:

Nurse Practitioner: $2,756.90 - $2,951.90 per week

Transitional Nurse Practitioner: $2,603.12 - $2,655.03 

We are seeking an experienced and dedicated Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) to join our Diabetes team.

This new role will contribute to achieving one of the key outcomes “Diabetes Prevalence in Adults” outlined in the NSLHD Service Level Agreement Health Outcomes.

The position will be targeted at identifying diabetes risk for pre- diabetes and Type 2 diabetes (T2DM), activate preventative measures and initiate early management of pre and T2DM. The key referral pathways to identify this population will be via inpatient and emergency department screening, a structured post-partum referral process for women with previous gestational diabetes and GP referrals through collaboration with the primary health network.

The role will provide clinical and professional leadership as an expert in the field as well as collaborating with other members of the health care team to plan, implement and evaluate care. This integrated approach will support best practice and maximise patient care outcomes.

Your scope may include initiating diagnostic investigations, prescribing medications, and making direct referrals to other health professionals. Guided by the values and knowledge of the nursing profession, you will contribute to innovative, flexible models of care that enhance and complement existing services.

Candidates will need to meet the following criteria:

  1. Transitional (Trainee) Nurse Practitioners must hold a current AHPRA registration as a Registered Nurse with endorsement to practice as a Nurse Practitioner or currently enrolled in an approved Nurse Practitioner Masters program working towards endorsement.
  2. Transitional (Trainee) Nurse Practitioners must have a minimum of 5,000 hours of advanced nursing/midwifery practice during the last six years appropriate to the speciality area (or working towards within the next 3 years).
  3. Working towards relevant post graduate tertiary qualifications, including masters of Nursing and an Australian Diabetes Educators Association (ADEA) accredited post graduate certificate in diabetes education.
  4. Demonstrated excellent collaborative, leadership, teaching and interpersonal skills.
  5. Highly developed evaluation and problem solving skills.
  6. Proven ability to work autonomously and as part of a multidisciplinary team.
  7. Commitment to quality improvement, practice development and evidence based practice.
  8. Ability to apply highly developed communication skills within the team environment and to patients and their families and/or carers.
