Community Aged Care and Rehabilitation Service at Ryde Hospital
What you'll be doing
Supporting older adults to live safely and independently at home.
- A Bachelor or post graduate degree in Social Work and eligibility for the Australian Association of Social Workers (AASW). Evidence of extensive clinical experience, inclusive of experience as a Level 2 Social Work. Current Class C Driver's Licence
- Demonstrated advanced clinical skills in comprehensive psycho-social assessments, counselling and therapeutic interventions, complex clinical casework, group work, advocacy and discharge planning.
- Demonstrated specialist knowledge and experience at a Level 2 or above in working with patients, families and carers with age related conditions.
- Demonstrated ability to work with minimal supervision and as a collaborative member of multiple interdisciplinary teams.
- Demonstrated ability to initiate, lead, complete and implement quality improvement and research activities.
- Demonstrated experience in the provision of supervision of social workers and social work students.
- Demonstrated well developed skills in written and verbal communication, computer literacy, time management, prioritisation, liaison and negotiation.
