About the role

  • St Vincent’s Hospital Transit Lounge is to provide an appropriate and safe environment for patients transiting into and out of St Vincent’s Hospital.

  • The Transit Lounge supplements inpatient capacity by being a space for patients who are ready for discharge but would otherwise occupy an inpatient or an ED bed. The Transit Lounge can also begin the new admission process for stable patients awaiting an inpatient bed.

  • As a Discharge Coordinator, you will play a pivotal role in ensuring smooth and effective transitions of care, particularly for patients with complex discharge needs. You will regularly be liaising with Medical Officers to ensure effective discharge planning and education for patients.

  • Collaborating closely with the multidisciplinary team, you will lead efforts to facilitate timely and safe discharges, making a tangible impact on patient outcomes and hospital efficiency.

  • Maintaining accurate patient records and contributing to service improvement initiatives.

About you

  • Current registration as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency (AHPRA).

  • Relevant post-registration qualifications and at least three years’ experience working in the clinical area of their specified post-graduate qualification.

  • Demonstrated understanding of principles, systems and processes related to patient flow and discharge planning.

  • Demonstrated understanding of operational factors that influence patient access and improve the patient journey.

  • Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.

  • Demonstrated leadership qualities and the ability to set priorities, establish and achieve agreed task and plans together with the ability to work with a broad range of health disciplines.
