What you'll be doing
The Sustaining NSW Families Program (SNF) Child and Family Health Nurse (CFHN) practices at an advanced level of nursing, implementing a sustained health home visiting program which commences in pregnancy and continues to the child's second birthday.  

The nurse works in a primary healthcare model with families who have been identified through the Safestart process. The SNF Nurse will perform health and development screening of infants/children in accordance with the Personal Health Record PHR schedule.  

The SNF Nurse also completes assessments as per the SNF program home visiting schedule using screening tools. The nurse works as part of a multidisciplinary team utilising case reviews to plan care. The SNF Nurse promotes, encourages and supports breastfeeding and the maintenance of lactation. A family partnership and anticipatory guidance approach is utilised to promote optimal infant and maternal mental health, child development and positive parent child relationships. 
  

About You
You are an experienced Child and Family Health Nurse and Midwife with current AHPRA registration and postgraduate qualifications in Child and Family Health. With extensive experience supporting children from birth to two years of age and their families, you are committed to delivering high-quality, family-centred care that promotes positive health and developmental outcomes.

You bring a strong understanding of the Sustaining NSW Families program and apply evidence-based practice to support vulnerable families within community settings. Your expertise includes providing clinical consultation, facilitating case reviews, and working collaboratively within multidisciplinary teams to develop effective care plans tailored to the individual needs of children and families. You are skilled at building therapeutic relationships and fostering productive partnerships with clients, government agencies and non-government organisations, drawing on your well-developed communication, interpersonal and conflict resolution skills.

You are confident in the use of electronic medical records, databases and other clinical information systems, ensuring accurate documentation and effective care coordination. Committed to continuous improvement, you have experience leading quality improvement initiatives and planning, coordinating and evaluating education programs that enhance outcomes for children and families. With a current NSW driver's licence and a willingness to travel, you are adaptable, proactive and dedicated to providing accessible and responsive community-based healthcare services.
