What you'll be doing
The Sustaining NSW Families Program (SNF) Child and Family Health Nurse (CFHN) practices at an advanced level of nursing, implementing a sustained health home visiting program which commences in pregnancy and continues to the child's second birthday.
The nurse works in a primary healthcare model with families who have been identified through the Safestart process. The SNF Nurse will perform health and development screening of infants/children in accordance with the Personal Health Record PHR schedule.
The SNF Nurse also completes assessments as per the SNF program home visiting schedule using screening tools. The nurse works as part of a multidisciplinary team utilising case reviews to plan care. The SNF Nurse promotes, encourages and supports breastfeeding and the maintenance of lactation. A family partnership and anticipatory guidance approach is utilised to promote optimal infant and maternal mental health, child development and positive parent child relationships.
About You
You present yourself as a clinician with deep, hands‑on expertise in Child and Family Health Nursing, grounded in your current authority to practise as a Registered Nurse and Midwife with AHPRA and strengthened by a post‑graduate Child & Family Certificate. You bring more than three years of post‑graduate experience working with children aged 0–2 and their families, drawing on extensive knowledge of early childhood development, family dynamics, and community health. Your familiarity with the Sustaining NSW Families Program allows you to connect your clinical background to evidence‑based home‑visiting practice, demonstrating how your experience aligns with the program’s goals of strengthening parent–child relationships and improving long‑term outcomes.
You show a clear capacity to provide clinical consultation and lead case reviews within a multidisciplinary, community‑based team, always maintaining a family‑centred approach. Your communication skills—both written and verbal—are a strong point, supported by confidence using electronic medical records, word processing tools, and databases. You work collaboratively and respectfully with clients, colleagues, government services, and NGO partners, using well‑developed interpersonal and conflict‑resolution skills to build and sustain effective therapeutic and professional relationships.
You also bring leadership experience, having guided a quality improvement project that enabled meaningful clinical change. Your ability to plan, coordinate, and evaluate education programs reflects your commitment to improving outcomes for families and supporting the professional growth of others. With a current NSW driver’s licence and a willingness to travel, you remain flexible and responsive to the needs of the role and the communities you serve.
