The Opportunity 

This role offers the perfect balance of autonomy and collaboration. As the Diabetes Educator and Cardiac Rehabilitation Clinical Nurse Specialist, you’ll join a multidisciplinary team delivering high-quality, individualised care across both community and acute settings.

If you’re earlier in your diabetes education journey, we still encourage you to apply. We support clinicians who are willing to complete relevant postgraduate qualifications within 12 months, with opportunities to progress to CNS2 classification as you build your skills and experience.


In this varied and rewarding position, you will:

  • Deliver specialised Diabetes Management and Cardiac Rehabilitation services

  • Provide individualised, case-managed care for clients with complex needs

  • Support both community-based and acute care services

  • Contribute to the MOICD Program, improving outcomes in Indigenous health

  • Play a key role in clinical leadership, education, and service development

  • You’ll also have the chance to make a real difference in surrounding communities through outreach clinics in Warialda, Bingara, Mungindi, Boggabilla, and Toomelah.

Additional Information 

  • Based on a minimum 18-month commitment to the position or the agreed duration of your contract. All payments to part-time and contract employees will be made on a pro-rata basis in accordance with the terms of your contract, including any approved extensions. If your contract ends before the agreed completion date, you may be required to repay a portion of the incentive. Please note: the increased RHWIS allowance is approved until 30th June 2026. Commencement after this date may be subject to change, including the allowance amount for which the candidate may be eligible.
