  • Contribute to the establishment and growth of a newly commissioned specialist nursing role within a tertiary healthcare setting. 
  • Deliver high‑quality, coordinated care through autonomous practice, advanced clinical reasoning, and sound decision‑making.
  • Work closely with a diverse multidisciplinary team to support integrated, patient‑centred care. 
  • Manage complex patient journeys, supporting continuity of care across inpatient, outpatient, and virtual settings.

You will bring advanced clinical capability, adaptability, and strong professional relationships to support high‑quality care within a specialist service. 

  • Possess advanced clinical knowledge to practise autonomously, with strong attention to detail. 
  • Demonstrate clinical case management experience with well‑developed clinical reasoning and decision‑making capability, while being comfortable and professional in patient interactions both in person and by phone, functioning as a PAH clinical resource. 
  • Are available to work within the PAH service during required clinical days, including participation in PAH MDT clinics. 
  • Are adaptable and able to perform a wide range of PAH‑focused duties within a multi‑faceted, PAH‑centric role, including clinical governance, administrative oversight and tasks, PAH service evaluation, and PAH research activities, with scope to contribute holistically to the service. 
  • Maintain collegial working relationships and effective communication with PAH service staff, administration officers, allied health personnel, inpatient teams and hospital doctors, community referring doctors, and other PAH centre nursing and medical staff in relation to shared‑care PAH patients. 
 

Selection Criteria 
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below. 

  • Demonstrated experience in delivering complex care to medical, respiratory, cardiology, and rheumatology patients, with the ability to apply these skills to the management of patients with Pulmonary Arterial Hypertension. This includes utilising advanced clinical knowledge, assessment, and decision-making to support coordinated, patient-centred care in collaboration with the multidisciplinary team doctors.
  • Demonstrate your ability to effectively communicate and engage with patient / consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs to the PAH patient.
  • Demonstrate how, as an CNS2, you will empower your PAH patients and their carers to optimise the management of their chronic condition.
  • Demonstrate how you will deliver a high-level capacity for patient advocacy, act as a change agent, and how you will evaluate services and adapt to change to new models of care.
