Working in close partnership with patients, families, carers, and a multidisciplinary team—including physicians, allied health professionals, and support staff—the CNS Grade 2 facilitates collaborative care planning to achieve clearly defined goals and optimal outcomes. This includes symptom management, psychosocial support, and end-of-life care coordination. The role also contributes to continuous improvement by mentoring staff, promoting evidence-based practice, and supporting education initiatives that enhance the overall quality and safety of the palliative care environment.
