Northern Sydney Home Nursing Service (NSHNS). Provision of service across NSLHD in home and clinic based settings.
What you'll be doing
The Clinical Nurse Specialist (CNS2) is a vital member of the NSHNS Continence Team, working under the clinical leadership of the CNC Continence, and closely supporting the Community Nursing Staff and Community based patients. They act as a role model providing complex care to patients requiring advanced clinical skills in Continence and related care. They also play a key part in leadership, advice, formulating and implementing policy, guiding practice and engaging in education and research activities.
NSHNS is a generalist Community Nursing Service with skilled registered nurses who visit clients in their own homes and in clinic environments for a range of care including comprehensive assessment, referrals to services and specialists depending on client need; IDC/SPC changes and support, continence assessments, NDIS assessments and reports, wound management – acute, simple to complex including compression therapy, NPWT, pressure injury management, conservative sharp wound debridement and vascular profiling etc; palliative care through to EOL care, syringe driver management, respiratory/heart failure monitoring, CVAD care and more.

Candidates will need to meet the following criteria:
- Division 1 Registered Nurse with post registration qualification relevant to speciality and at least 3 years experience working in the clinical area of their post graduate qualification or such other qualifications or relevant equivalent experience.
- Demonstrated advanced clinical knowledge, skills and experience in Continence management, with a commitment to evidence-based practice, patient-centred care and quality improvement. Must have current Assessor Competency (either LHD Competency, or Cert IV Training and Assessment).
- Proven ability to lead clinical practice through education, mentorship and the implementation of initiatives that enhance wound care service delivery and patient outcomes.
- Highly developed communication and collaboration skills, demonstrated ability to work effectively within multidisciplinary teams, proficiency in Microsoft applications, and possession of a current NSW Driver Licence.
