About you:
The ICCAPP team are seeking a dedicated individual who is passionate about providing holistic, culturally appropriate health services for Aboriginal people and their families.
- Excellent communication skills with a strong focus on patient care.
- Exceptional work ethic and strong time management skills.
- Ability to work effectively with Aboriginal people within their community to provide holistic care.
- Confidence to work independently within your scope of practice while knowing when to seek support
- Experience or strong interest in diabetes and/or cardiology health, with a deep understanding of chronic disease management & patient care strategies.
Essential criteria:
- Current Authority to Practice with AHPRA who has at least 5 years full time equivalent post registration experience.
- Post graduate qualification in chronic disease or evidence of working towards same and at least five years post graduate nursing experience with 3 years' experience in chronic disease management.
- Current NSW Driver's License and capacity to travel for work.
