About the Opportunity
We are seeking an experienced Clinical Nurse Consultant – Chronic Heart Failure to deliver expert, patient-centred care for people living with heart failure across Murrumbidgee Local Health District. In this advanced practice role, you will provide specialist consultancy, coordinate complex care, and work with multidisciplinary teams to improve patient outcomes and support effective chronic disease management.
The Clinical Nurse Consultant (CNC) provides high-level clinical expertise and leadership across the five CNC domains: clinical service and consultancy, clinical leadership, education, research, and clinical service planning and management.

 

About the Opportunity
As the CNC – Chronic Heart Failure–, you will be responsible for:

  • Providing advanced clinical assessment and consultancy for patients with complex chronic heart failure and chronic disease management .
  • Developing, implement and evaluate individualised care management plans in partnership with patients, carers and multidisciplinary teams.
  • Providing expert advice and clinical leadership to nursing staff, and other healthcare professionals.
  • Leading the development and implementation of evidence-based cardiac care practices across services.
  • Delivering specialist education and support to patients, families and health professionals to improve chronic disease self-management.
  • Contribute to clinical research, quality improvement and service innovation within cardiac care.
  • Participating in, strategic planning and service development to enhance cardiac care services, across Murrumbidgee Local Health District.

About You
Our ideal candidate will demonstrate:

  • Registered Nurse with current AHPRA registration
  • Demonstrated experience in chronic cardiac or heart failure management
  • Strong clinical assessment and decision-making skills
  • Proven ability to provide clinical leadership and specialist consultancy
  • Excellent communication and collaboration skills within multidisciplinary teams 
  • Commitment to evidence-based practice, quality improvement and patient-centred care
  • Passion for improving outcomes for people living with chronic heart failure
  • A strong understanding of the five Clinical Nurse Consultant domains
