The Clinical Nurse Consultant (CNC)provides expert clinical advice to patients/clients, carers and other health care professionals with complex healthcare needs within Palliative Care. The Clinical Nurse Consultant is a clinical expert who, through leadership, facilitates excellence in research based clinical practice, education, research and professional leadership. The Clinical Nurse Consultant models the CORE (Collaboration, Openness, Respect and Empowerment) values of the organisation.
The CNC is part of a wider multidisciplinary team providing care to patients within Community Palliative Care Specialty and the wider health service. The CNC will work as part of a team to develop, facilitate, and evaluate the implementation of care management plans for patients with a focus on care coordination for patients identified with complex needs, EXAMPLE Those patients that require more specialised assessment and management skills than the generalist nursing scope.
The CNC will provide expert consultation and advice to other services within the ISLHD, community-based services, health care workers for patients with special or complex needs through their continuum of care, acknowledging that, ensuring support is provided to the patient from diagnosis through treatment ensuring quality end of life care.
The CNC will establish and maintain communication with all relevant Medical Practitioners, Nursing and Allied Health Professionals, will assist in the organisation and participate in the multidisciplinary team meetings, and maintain a close liaison with all Division of General Practitioners, Community GP's, Residential Aged Care Facilities and Community Health Services. The CNC will engage in a community of practice and supervision.
