What you'll be doing 
In this influential clinical leadership position, you will provide expert consultation, leadership and coordination to support patient-centred continuing care, discharge planning and wound management services across Auburn Hospital. You will play a pivotal role in advancing evidence-based practice, quality improvement and contemporary models of care.

·         Lead care coordination, discharge planning and patient flow initiatives to support safe, effective and seamless transitions of care

·         Provide expert clinical consultancy and specialised wound management expertise to clinicians, patients and multidisciplinary teams

·         Drive the development, review and implementation of clinical pathways, policies, procedures and best practice standards

·         Lead quality improvement, accreditation, research and workforce development initiatives that strengthen patient outcomes and service delivery

·         Analyse clinical data, monitor key performance indicators and provide strategic advice to support service planning and decision-making

Essential Criteria:

·         Qualification: Postgraduate Acute Care qualifications relevant to the field 

·         Registration: Current and valid registration as a Registered Nurse (Division 1) with the Nursing and Midwifery Board of Australia 

Selection Criteria: 

1.     At least five (5) years full time equivalent post registration experience, with at least 3 years full time equivalent within an acute care setting. Holding or working towards relevant postgraduate tertiary qualifications in a specialty area, education and/or research.

2.     Demonstrated advanced communication including verbal, written and interpersonal skills with the ability to effectively engage with clients/consumers, carers and the multidisciplinary health team in a manner than ensures responsiveness of care needs and resolves conflicts when they arise. and advanced information technology skills to provide consultancy services and deliver innovative nursing practice

3.     Demonstrated ability to work effectively and collaboratively both independently and as part of the multidisciplinary health care team.

4.     Demonstrated ability to lead quality improvement, practice development and evidence-based practice critical review

5.     Demonstrated personal attributes including resilience, accountability, integrity, and ability to manage self.

6.     Demonstrated ability to identify education needs and deliver education for patients/clients and health care professionals

7.     Demonstrated ability to function as a senior leader within a complex health organisation.

8.     Demonstrated ability to use critical thinking and problem-solving skills as it relates to the delivery of advanced evidence based acute and chronic care nursing.
