About the Opportunity 
The Renal Supportive Care CNC provides supportive care services to Renal patients in SWSLHD. The role assists in the clinical management of patients with CKD in conjunction with the RSC Nursing team, RSC Allied Health Team, RSC Medical team and the referring Nephrologist. This position requires care to be delivered across renal services in inpatient, outpatient, and community health settings.

The role will act as a clinical leader and expert practitioner providing specialty related education for health professionals, fostering an environment of continual learning, and skills including maintenance and coordination of renal policies for the district.

This position will work in collaboration with the Director of Renal Medicine, Nephrologists, Renal Nurse Manager, Nursing teams, allied health and community care providers to identify, conduct, facilitate and promote research projects to enhance best patient outcomes.

Regular liaison will occur with the Renal Service Director, Renal Nurse Manager, relevant Nurse Unit Manager’s and Clinical Nurse Educator’s in each department to identify staff educational needs and develop strategies for the education of staff, consumer groups, patients and others.

The CNC will promote a service which fosters clinical excellence, research and professional leadership. The CNC contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes. The CNC is responsible for providing services which facilitate effective management of the care in the given specialty.

  • Renal Nurse Manager SWSLHD
  • Director of Renal Service SWSLHD

Strategically linked to the Cardiovascular Stream through the Renal Service

How to Apply

  1. Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA). At least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field. In addition, the employee must have approved postgraduate nursing/midwifery qualifications relevant to the field in which the employee is appointed, or such other qualifications or experience deemed appropriate by the public hospital or public health organisation 
  2. Extensive Experience in Dialysis or Palliative Care Nursing
  3. Can you provide examples of how you maintain up-to-date knowledge of clinical practice and research in your specialty, and how you apply this expertise to deliver culturally responsive, evidence-based, and safe patient care?
  4. Describe a situation where you effectively communicated with patients, carers, or a multidisciplinary team to meet care needs, demonstrated critical thinking, and contributed to conflict resolution, risk management, or quality improvement in the workplace.
