Registered Nurse: $41.22 - $57.88 per hour

Nepean Hospital is seeking a compassionate and experienced Clinical Aged Care Assessor to join our dedicated team. In this rewarding role, you will work closely with older people, their families, carers, and healthcare providers to assess care needs and support access to appropriate aged care services that promote independence, wellbeing, and quality of life.
As part of a multidisciplinary team, you will conduct comprehensive assessments, develop person-centred care recommendations, and contribute to positive healthcare outcomes for older members of our community. The Clinical Aged Care Assessor is responsible for undertaking holistic assessments of older people across hospital and community settings, identifying support needs, and facilitating access to aged care programs and services. You will collaborate with a range of stakeholders to ensure older people receive timely and appropriate care tailored to their individual circumstances

What you will bring to the role
- For Nursing/Occupational Therapist/Physiotherapist - Relevant tertiary qualifications in Nursing/ Occupational Therapy/or Physiotherapy with full AHPRA Registration; OR For Social Worker/Dietitian/ Speech Pathologist - Bachelors degree or post graduate degree in Social Work/ Dietetics/or Speech Pathology providing eligibility for membership with the relevant Australian peak professional body for the relevant discipline.
- Proven high standard of written and verbal communication skills. Written skills need to be precise, clinically sound and completed in a timely manner. Proven ability to use Microsoft Office Programs, electronic patient record systems and e-Learning sites.
- Demonstrated discipline and clinical experience in relevant aged care settings, with a proven ability to conduct comprehensive assessments, develop and implement tailored care plans that address the needs of older individuals.
- Demonstrated knowledge of My Aged Care, related aged care programs and services with proven ability, to use this knowledge to assess and support both routine and complex aged care presentations.
- Proven clinical skills in the assessment of dementia and other age-related diseases.
- Proven ability to work independently as a clinician and collaboratively as a member of multidisciplinary team and the ability to work flexibly across and within all ACA sites.
- Current unrestricted Driver's Licence, with a willingness to travel in accordance with the demands of the position.
