  • This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life.

     

  • It is a community health centre based position requiring travel across sites and into private residences.

     

  • This position is required to participate in afternoon, weekend and public holiday shifts.

     

  • The rotation to these shifts is minimal at this current time, and is subject to change at anytime.

What you'll be doing 
As a Chronic & Complex Community Registered Nurse, you will provide comprehensive nursing assessment, care coordination and evidence-based clinical interventions to support clients living with chronic and complex health conditions. Working both independently and collaboratively, you will promote health, enhance quality of life and support clients to remain safely at home.

·         Deliver client-centred nursing assessments, care planning, care coordination and ongoing clinical management

·         Monitor, educate and support clients, carers and families to achieve optimal health outcomes and self-management

·         Collaborate with multidisciplinary teams to provide integrated and holistic care across the continuum of care

·         Complete accurate clinical documentation, case reviews and discharge planning to support safe and effective care delivery

·         Apply contemporary nursing practice, clinical judgement and problem-solving skills to manage complex healthcare needs

Essential Criteria:

  • Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)

3.     Demonstrated effective communication skills both written and verbal. Effective interpersonal skills showing initiative and leadership.

4.     Demonstrated commitment to professional development, evidence-based practice, and an awareness of changes within the health care environment.

5.     Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.

6.     Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.

7.     Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training

8.     Current Drivers Licence valid within Australia and willingness to use for work purposes.
