This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life.

It is a community health centre-based position requiring travel across sites and into private residences. This position is required to participate in afternoon, weekend and public holiday shifts.

The rotation to these shifts is minimal at this current time and is subject to change at any time.

What you'll be doing 
As a Chronic & Complex Community Registered Nurse, you will provide comprehensive nursing assessment, care coordination and clinical interventions to clients living with chronic and complex health needs. Working both independently and as part of a multidisciplinary team, you'll play a vital role in supporting clients and their families to maintain and improve their health and quality of life.

Key responsibilities include:

·         Conducting comprehensive assessments and developing person-centred care plans tailored to individual client needs

·         Delivering evidence-based nursing care, health education, monitoring and ongoing clinical management in the community

·         Coordinating care with clients, carers, families and healthcare professionals to achieve optimal health outcomes

·         Maintaining accurate clinical documentation, participating in case reviews and contributing to quality improvement activities

·         Promoting client safety, wellbeing, dignity and independence while applying contemporary nursing practice standards

Essential Criteria:

·         Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)

8.     Current Drivers Licence valid within Australia and willingness to use for work purposes.
