At Central Coast Local Health District (CCLHD), we are committed to delivering Trusted Care and Better Health for Everyone and we are looking for an experienced Senior Social Worker (Level 3) to join our Community Allied Health team.


About the Opportunity

The Senior Social Worker is part of a community-based multidisciplinary allied health team delivering services to clients and families across the Central Coast. You will work within a dedicated interdisciplinary team environment and collaborate closely with specialist services including Transitional Aged Care Programs, Specialist Palliative Care Services, Chronic Disease Programs and Integrated Care Coordination teams.

This role combines advanced clinical practice with leadership, workforce coordination, and service development responsibilities. Social Work services are delivered across community settings including home visits, clinics, and group programs. Regular travel across the Central Coast region is required.

As community Allied Health services continue to expand across the Central Coast, there may be opportunities for weekend service delivery in the future (not currently available).

In this role you will:

Provide Expert Clinical Care

  • Deliver evidence-informed assessment, counselling, case management, therapeutic intervention, liaison, advocacy, and referral for clients experiencing acute and chronic health conditions.
  • Support patients and carers to manage the emotional and psychosocial impact of illness.
  • Undertake comprehensive case planning to ensure timely access to appropriate community supp

Lead and Develop Others

  • Provide professional supervision, mentoring, and coaching to Level 1/2 Social Workers and students.
  • Participate in performance appraisal and professional development planning processes.
  • Support workforce coordination and allocation of Social Work services across community sites to ensure effective and timely service delivery.
  • Model best practice and foster a positive, collaborative team culture.
     

Contribute to Multidisciplinary Excellence

  • Actively collaborate within the community Social Work team, the broader multidisciplinary team, and the CCLHD Social Work department.
  • Work effectively with Allied Health Assistants and interdisciplinary colleagues to deliver person-centred, coordinated care.
  • Demonstrate commitment to the Caring for the Coast vision and uphold NSW Health CORE values and CCLHD Values and Behaviours in all aspects of practice.

Drive Service Innovation

  • Contribute to service development, evaluation, and quality improvement initiatives.
  • Participate in research activities and allied health projects across CCLHD.
  • Engage in staff and student teaching and professional education activities to strengthen workforce capability.

About You

You are a confident and experienced social work professional who brings:

  • A recognised Social Work degree with AASW eligibility (including overseas assessment where relevant).

     

  • Extensive clinical experience across community health settings,including complex case management.
  • Demonstrated leadership capability including supervision, mentoring, and performance development.
  • Advanced skills in psychosocial assessment, therapeutic intervention, and group facilitation.
  • Excellent communication, negotiation, and advocacy skills.
  • The ability to manage competing priorities in dynamic environments.
  • A current NSW Driver’s Licence and eligibility to work in Australia.
