CCLHD - Registered Nurse - Palliative care service

The Central Coast Local Health District (CCLHD) is seeking a highly motivated and clinically experienced Registered Nurse to join our Specialist Palliative Care Service, based at the Long Jetty Health Care Campus. This is an excellent opportunity to be part of a dedicated, multidisciplinary team delivering high-quality, person-centred care to patients and families across a range of care settings within the Central Coast region.
About the Role
The successful candidate will provide comprehensive palliative nursing care across acute, community, and residential aged care settings. This includes care for inpatients referred to the consultation service, clients at home or in residential aged care facilities (RACFs), and supporting intake and triage functions.
This position operates primarily Monday to Friday from 08:30 to 17:00, with participation in a rotating after-hours on-call roster (17:00 to 20:30). Staff may also be rostered to cover a day shift on weekends approximately every 4–5 weeks, depending on leave and service needs. There will be a 6 week orientation program provided.
You will work collaboratively with an experienced, multidisciplinary team to provide coordinated, compassionate care that supports patients and their families through end-of-life care, including bereavement support and advanced care planning.
Key Responsibilities

Provide advanced, holistic nursing care in alignment with the principles of palliative care and within the Registered Nurse scope of practice

Conduct assessments, develop care plans, implement evidence-based interventions, and evaluate care outcomes

Communicate effectively and compassionately with patients, carers, and other health professionals to ensure coordinated and high-quality care

Maintain accurate and timely clinical documentation using Microsoft Office and electronic medical record (eMR) systems

Participate in continuous quality improvement, service development, and patient safety initiatives

Contribute to team-based learning and participate in professional development and performance review processes

Adhere to all relevant CCLHD and NSW Health policies, procedures, and standards, including ethical and professional conduct frameworks

About You:
- Current registration as a Registered Nurse with AHPRA
- Demonstrated experience or a strong interest in Palliative Care
- Current Australian driver’s licence and willingness to travel across the Central Coast
- Excellent communication, clinical reasoning, and problem-solving skills
