The Central Coast Local Health District (CCLHD) is looking for a Registered Nurse - Priority Populations to provide patient centred care to children and young people in statutory care referred to the program. This role works within a multidisciplinary team to provide timely review, coordination, referral, and assessment as required for children and young people (0-18 years) in statutory Out of Home Care.
About The Opportunity
This position is a key team member of the Out of Home Care Health Pathway Program (OOHC-HPP). The OOHC HPP is a collaboration between NSW Health, Department of Communities and Justice, OOHC service providers, Department of Education, carers, and children and young people themselves. Located at Erina Community health Centre, this niche team includes a Coordinator, Clinical Nurse Specialist, Registered Nurse (this role), Occupational Therapist, Speech Pathologist and Administration Officer.
The RN works within the team to undertake the assessment, planning, implementation, and evaluation of nursing interventions to achieve health goals. A significant focus of the role is collating and reviewing health care information in relevant electronic systems to ensure necessary information is available to meet legal requirements and to guide ongoing care. The role operates collaboratively alongside patients, carers, other healthcare providers, and external partners, particularly DCJ and NGO agencies. The team complete, review or coordinate primary health assessments and develop a Health Management Plan. This Health management Plan supports young people as they come into statutory care, through significant life transition points and as they plan to leave care.
This role is based at Erina Community Health Centre with travel across the Central Coast required.

About You
- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) with post registration qualifications in a relevant field within Paediatrics, Child and Family Health, or Adolescent/Youth health.
- Demonstrated knowledge of the effects of adverse childhood experiences (ACEs) on child hood development.
- Demonstrated experience in coordinating follow up health care for children and or young people.
- Demonstrated ability to work effectively and collaboratively as part of the multi-disciplinary health care team.
