About the Opportunity
As primary health care workers in the community, nurses are required to be involved in the continuous monitoring of the health status of communities as well as responding to episodic health needs. Because of the settings in which it is delivered, community nursing requires skilful application and adaptation of the scientific principles on which nursing is based.
The focus of community nursing is on the promotion and maintenance of health. It is also concerned with early recognition of and prompt intervention in the disease process and with rehabilitation activities, all of which aim to prevent disease, retard its progress, minimize its effects and assist the individual to achieve and maintain an optimum level of independence, or a peaceful death when life can no longer be sustained.
Community Nurses, recognising the importance of social and ecological influences on the health needs of the individual, accept the responsibility to ensure that community nursing is relevant by developing responsive strategies to enable them to adapt to the changing health needs of the community.
In this role you will:
Collaborate with other health care providers, provide optimum generalist nursing services to clients and the community of the Central Coast.
Provide clinical/physical assessment, care planning, case management and documentation, clinical procedures, provision of care, support, education and advice to client and carers.
Record data for the evaluation of the service.
Communicate effectively with patients, families, and the healthcare team to promote a supportive environment.

About You
We are looking for someone who has:
Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) (Division 1)
Demonstrated high level communication skills including negotiation and interpersonal skills.
Recent nursing experience in Community Nursing or a demonstrated interest in Community Nursing.
Demonstrated high level time management skills and an ability to effectively work both within a team and autonomously.
Demonstrated ability to liaise with other health professionals to facilitate case management and care planning.
