This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks’ notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.

About the Role

As a Community Nurse, you’ll deliver high-quality nursing care in people’s homes and community settings — promoting health, supporting independence, and responding to acute, chronic, and complex health needs.

This role combines clinical skill, critical thinking, and compassionate care with health promotion, early intervention, and rehabilitation.

Your responsibilities will include:

  • Monitoring the health and wellbeing of individuals, families, and the broader community
  • Providing patient-centred care for acute, chronic, palliative, and post-acute conditions
  • Promoting self-management, wellness, and recovery through education and support
  • Identifying early signs of disease or deterioration and intervening appropriately
  • Advocating for clients and supporting access to relevant services and resources
  • Responding to the diverse social, environmental, and cultural needs of the population
  • Collaborating closely with multidisciplinary teams, carers, and local services
  • This is a temporary full-time position, working Monday to Friday, 8:00am to 4:30pm – no weekend or night shifts required.

About You

  •        Registered Nurse (Division 1) with AHPRA, with current registration and hold a current Australian Drivers Licence.
  •        Recent nursing experience in Community Nursing or a demonstrated interest in Community Nursing.
  •        Demonstrated ability to liaise with other health professionals to facilitate case management and care planning.
  • Strong clinical reasoning, assessment, and documentation skills
  • Excellent communication, problem-solving, and time management
  • A commitment to health promotion, equity, and responsive care
