Join the Community Nursing team at Central Coast Local Health District (CCLHD), where your expertise will help individuals live independently, manage chronic health conditions, recover from illness, and maintain dignity in their own homes.

In this rewarding role, you will deliver a broad range of clinical services including wound care, palliative care, catheter and continence management, CVAD care, and drain management.

About the Role

In this role, you will:

  • Deliver high-quality, evidence-based nursing care in clients’ homes, community clinics, and outreach settings, ensuring each person receives safe, effective, and compassionate care that meets their individual needs.

  • Lead and participate in health promotion and preventative care initiatives across the community, such as education sessions, screenings, and wellness programs, to improve health outcomes and reduce preventable hospital admissions

  • Build strong, trusting relationships with clients and their families, providing clear health information, practical guidance, and emotional support to empower them to take an active role in managing their health and wellbeing.

  • Collaborate closely within a multidisciplinary team – including allied health professionals, GPs, and community agencies – to design and implement integrated care plans that address physical, emotional, and social health needs

About You

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.

  • Competent clinical skills aligned with the Registered Nurse Standards of Practice.

  • Strong abilities in the assessment, planning, implementation, and evaluation of patient care.

  • Ability to delegate and supervise Enrolled Nurses (ENs) and Assistants in Nursing (AINs) within their scope of practice.
