About the Role
As a Registered Nurse within the Community Cardiac Services team, you will provide high-quality, person-centred nursing care to patients managing cardiac conditions. This role focuses on supporting recovery, preventing hospital readmissions, and empowering patients through education, self-management strategies, and early intervention.
Working collaboratively with a multidisciplinary team, you will perform clinical assessments, monitor patient progress, coordinate care, and contribute to the development of individualised care plans. This is a rewarding opportunity to make a meaningful impact in the lives of patients while promoting heart health across the Central Coast community.
This role is a Part-time position, weekdays 8:00am to 4:30pm (no weekends)

About You
- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Recent clinical experience in Cardiac Services and commitment to quality improvement, practice development and evidence-based practice.
- Undertakes nursing interventions to the level and scope of practice, to support the patient/client to achieve their health and wellbeing goals.
- Coordinates nursing and health care to facilitate positive health outcomes.
