 Join the Central Coast Local Health District (CCLHD) as a part-time Registered Nurse in our Community Cardiac Services team, where you’ll play a vital role in delivering expert care to cardiac patients across the community.

  

About the Role

As a Registered Nurse within the Community Cardiac Services team, you will provide high-quality, person-centred nursing care to patients managing cardiac conditions. This role focuses on supporting recovery, preventing hospital readmissions, and empowering patients through education, self-management strategies, and early intervention.

Working collaboratively with a multidisciplinary team, you will perform clinical assessments, monitor patient progress, coordinate care, and contribute to the development of individualised care plans. This is a rewarding opportunity to make a meaningful impact in the lives of patients while promoting heart health across the Central Coast community.

This role is a Part-time position, weekdays 8:00am to 4:30pm (no weekends)

About You

  •        Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  •        Recent clinical experience in Cardiac Services and commitment to quality improvement, practice development and evidence-based practice.
  •        Undertakes nursing interventions to the level and scope of practice, to support the patient/client to achieve their health and wellbeing goals.
  •        Coordinates nursing and health care to facilitate positive health outcomes.
