Central Coast Local Health District is seeking a dedicated and compassionate Physiotherapist to join our multidisciplinary team within the Transitional Aged Care Program (TACP). This role delivers essential physiotherapy services across both community (Transpac) and residential (Transitional Care Unit) settings, supporting clients transitioning from hospital care to long-term care solutions.
As a Physiotherapist in TACP, you will work closely with patients, families, and carers to:
• Complete the restorative process post-hospitalisation
• Optimise functional independence and mobility
• Assist clients in accessing and establishing appropriate long-term care arrangements
Key Responsibilities:
• Deliver evidence-based physiotherapy interventions as part of a multidisciplinary team
• Conduct assessments and develop person-centred care plans
• Monitor patient progress and adjust treatments to meet evolving goals
• Support patient empowerment through education and guided therapy
• Collaborate with other health professionals to ensure coordinated, holistic care

About You:
• Tertiary qualifications in Physiotherapy and current AHPRA registration
• Demonstrated clinical experience in aged care, rehabilitation, or community physiotherapy
• Have a current or willing to obtain a Aged Care Check
• Ability to work as an effective part of a multidisciplinary team
