Join our dedicated Sustaining NSW Families Home visiting team - Central Coast Local Health District (CCLHD) and play a vital role in supporting children and families who may be open to a little more … supporting childhood development, family wellbeing, and parenting confidence.

 About the Role

Our nurse led program provides identified families with an opportunity to achieve their developmental, attachment and parenting goals over an extended time. The program begins in the Antenatal period and extends to the child’s 2nd birthday. The nurse works in partnership with families in their home, within a collaborative model of care, supported by a multidisciplinary team of allied health professionals and the broader Child and Family Health service.

About You:

You are:

·         A reflective practitioner, who is passionate about the First 2000 Days initiatives, working with families, early intervention and creating positive outcomes for parents and children

·         Have child and family health nursing competence and an ability to practice at an experienced level of nursing. This includes a detailed knowledge of child growth, development and perinatal and infant mental health.

·         Have an ability to work with a strengths based, partnership approach and a trauma informed lens, to develop therapeutic relationships with families.

·         Able to work in a culturally safe capacity.

·         Are willing to complete the SNF advanced nurse practice training.

You will bring:

  • Current AHPRA registration as a Registered Nurse and a valid NSW Driver’s Licence.
  • Knowledge, skill, optimism, warmth and hope working with the families and children in your Caseload. 
  • Ability to identify, address and respond appropriately to safety concerns for infants and children, and responding to instances where an infant or child is at risk of significant harm (ROSH).
  • Excellent communication skills – verbal, written, and through digital health systems.
  • A collaborative approach to working within a multidisciplinary team.
