CCLHD - Care Navigator – Paediatric Behavioural Triage

Central Coast Local Health District (CCLHD) is seeking a compassionate and skilled Care Navigator to join the Paediatric Behavioural Triage. Working within the Women Children and Families Directorate, this role is essential in helping children, young people, and their families access appropriate care and support pathways.
As a Care Navigator, you will play a key role in improving access to services for children and young people (and their parents/carers) with suspected co-morbid neurodevelopmental, behavioural, and mental health issues. You will:
- Provide coordinated ‘service navigation’ support, ensuring families are connected to the most appropriate services.
- Collaborate closely with clinicians, NGOs, and Community Managed Organisations (CMOs) to establish and maintain referral pathways.
- Support families across various care settings, including hospitals, community services, and through telehealth and telephone-based engagement.
- Facilitate a smoother experience for families navigating the health and social service systems.
- Contribute to the development of culturally safe and accessible services for all families.
This role is suitable for:
- Clinical Nurse Specialist Grade 2
- Dietitian Level 3
- Occupational Therapist Level 3
- Physiotherapist Level 3
- Social Worker Level 3
- Speech Pathologist Level 3
- Tertiary qualification in Health Sciences, Social sciences, Allied Health, Nursing or demonstrated industry experience with certificate IV in Mental Health. Please upload a copy of your qualification(s) to your application.
- Experience working in health, mental health, or social/community services, ideally with children, young people, and families.
- Strong understanding of neuro developmental, behavioural, and mental health challenges in children and adolescents.
- Knowledge of local service systems and the ability to collaborate across sectors (government, NGO, CMO).
