About the Role
Concord has built on its strengths of providing quality health services to returned servicemen and women and local communities, as well as undertaking world class clinical education and research.
The SLHD Community Transition Care Program is a goal-oriented program designed to assist eligible hospital inpatients to return to living at home safely, by providing low intensity therapies and assistance with activities of daily living for up to 12 weeks. The position is a community role, covering the Sydney Local Health District geographical area. TACP has its office base at Concord Hospital, but you may be asked to commence or finish work from other SLHD locations which include RPA, Canterbury and Balmain Hospitals.
The Transition Care Case Manager ensures that local program therapies, services, documentation, communication, and administrative requirements meets the needs of the client, and relevant SLHD, NSW Ministry of Health, and Commonwealth Transition Care guidelines.
Ideal Candidate
Current NSW Drivers Licence (P1 and P2 acceptable).
Tertiary qualifications in Nursing or Allied Health (specifically Occupational Therapy, Physiotherapy, Speech Pathology, Social Work or Dietetics) registration with AHPRA or membership to appropriate professional association if required.
Experience in assessment, care planning, monitoring and review of aged care clients.
