About the Role 

Concord Repatriation General Hospital has a well-earned reputation for excellence in healthcare for over 70 years. Building on its proud heritage of caring for the Veteran community, the hospital now services the local communities of Concord, Strathfield, Burwood and beyond.

 

The Out of Hospital Care (OHC) service incorporates the ComPacks, Safe and Supported at Home (SASH) and End of Life (EoL) programs. The OHC service supports hospital discharges and prevents avoidable admissions by providing short and medium-term packages of non-clinical care. The Case Manager role focuses on assessing clients for the program, developing a care plan of required community supports and providing ongoing case management.

Ideal Candidate

 

  • Tertiary qualifications in Nursing or Allied Health (specifically Occupational Therapy, Physiotherapy, Speech Pathology, Social Work or Dietetics) registration with AHPRA or eligibility for membership to appropriate Professional Association if required.

  • Experience in the comprehensive assessment, care planning, review and monitoring practices in community care service provision.

  • Experience in service provision for clients who are elderly, disabled, with chronic or complex care needs or requiring end of life care; with knowledge of the delivery and monitoring of brokered services.

  • Current NSW Drivers Licence (P1 and P2 acceptable).
