Join the Cardio-Pulmonary Rehabilitation multidisciplinary team and support people to build the knowledge, confidence and skills they need to improve their health, manage chronic conditions and reduce their risk of future cardiac and respiratory complications.

Based at the new Cardio-Pulmonary Unit in Chatham, you'll work alongside experienced Clinical Nurse Consultants, Chronic Care Support Nurses and Physiotherapists to deliver individualised rehabilitation programs for people recovering from cardiac events or living with respiratory illness.

You'll also support outreach services in Forster, connecting with patients in the Community Health setting, helping them achieve meaningful health and wellbeing goals.

About the role

As a Cardio-Pulmonary Rehabilitation Registered Nurse, you'll provide person-centred, evidence-based nursing care that promotes positive health outcomes.

You'll work closely with patients to:

• Assess, plan, implement and evaluate individualised nursing care

• Support patients following cardiac events and those managing respiratory conditions

• Encourage lifestyle modification to reduce future cardiac risk

• Help patients develop practical skills to manage respiratory-related conditions

• Identify and manage patient risk factors in collaboration with the multidisciplinary team

• Provide clinical education to support patients in managing their health

• Contribute to individualised exercise and rehabilitation programs

• Apply clinical reasoning, critical thinking and evidence-based practice to your nursing care

• Cardiac Rehabilitation Clinical Nurse Consultant

• Pulmonary Rehabilitation Clinical Nurse Consultant

• Chronic Care Support Registered Nurses

• Physiotherapists

• Nurse Unit Manager

You'll be supported by experienced clinicians who are committed to providing high-quality rehabilitation and helping you succeed in the role.

What we're looking for

We're seeking a highly skilled and experienced Registered Nurse who is passionate about helping people achieve better health outcomes.

You'll bring:

• Current registration as a Registered Nurse with AHPRA

• Clinical experience in cardiac and pulmonary care

• Experience in, or an understanding of, cardiac and pulmonary rehabilitation

• Demonstrated understanding of evidence-based practice and critical thinking

• Excellent communication and interpersonal skills

• The ability to deliver individualised, person-centred care

• A caring, resilient and motivated approach

• A strong commitment to teamwork and positive patient outcomes

You'll need to be someone who can work cohesively within a multidisciplinary team while also building trust and confidence with patients as they work towards their individual goals.
